All physicians, physician groups and physician organizations who provided covered services to any plan member or any other individual enrolled in or covered by a plan offered or administered by any person or entity named as a defendant between January 1, 1997 and December 31, 2006, is eligible for reimbursement from the Settlement Fund of $128 million dollars.The amount each healthcare provider receives is dependent on the overall number of claims submitted.
